12th Annual User Group – 22nd - 23rd November 2004

A highly successful meeting was held by all accounts with over 100 delegates attending.  It is hoped to publish the PowerPoint slides on our web site within the next few weeks and to publish a Proceedings document in January 2004.

Request from The UK National Patient Safety Agency (NPSA)

Prof David Cousins, Head of Safe Medication Practice at the National Patient Safety Agency gave a very informative presentation at the User Group. His presence at the meeting also added greatly to the discussions regarding the safe operation of anticoagulation services. 

Prof Cousins reported, “There have been 480 reported cases of harm or near harm from the use of anticoagulants in the UK from 1990-2002. In addition there have been 120 deaths reported over the same time period.”

Consequently the NPSA see anticoagulants as one of the top medication risks in the UK and currently have a project studying what needs to be done to improve medication management.

Prof Cousins is very interested in receiving examples of dosing report forms where centres have decided to move away from the Yellow Book. If you could help we can supply you with his address.

Upgrade from Version 4 and 5

Every three to four months we are making new releases of Dawn AC with enhancements to improve the safety and efficiency of anticoagulation care. Currently we anticipate version 6.09 to be available early in the New Year.

We feel that it is extremely important to take advantages of these upgrades because of the improved safety of operation they offer. Please contact us to discuss your position if you need to upgrade.

Benchmarking Questionnaire Report – Huge Variation in Practice

One of the key steps in benchmarking is to define the process being used. A process is a series of interrelated tasks that are organised to produce an output. By studying the processes of the highly successful centres in terms of say ‘time in range’ we might learn how to improve our own performance or at least explain the differences.

To help define the processes and understand the variation in practice I constructed a comprehensive questionnaire with 69 questions addressing nine areas of an anticoagulation service.

With the help of and thanks to 30 Benchmarking Service Customers completed the survey.

The Customers were grouped into three groups – the top 25%, Middle 50% and bottom 25% in terms of their % Time in Range results.

Each questionnaire answer is then displayed showing the practice against these three groups. There are some interesting patterns of practice among the top group customers, but the striking thing is the variation in practice.

This is probably the most detailed analysis of Anticoagulation Care practice to date.
